We are a healthcare innovation, mission-driven company that envisions to solve teamwork barriers for physicians, practitioners and other administrative leadership in healthcare systems. Our Chief Medical Officer, Dr. Arif Nazir an experienced physician and medical director in the post-acute setting struggled to keep a seamless connection with his facility leadership regarding key activities such as facility quality assurance, staff education and many other critical tasks. As a result his team missed many opportunities to enhance the care for their patients.
Soon he realized that challenges he faced were not unique, and were just a fraction of many other challenges that other healthcare teams faced. Particularly, in the post-acute settings where complex patients require a multi-faceted efforts to successfully achieve their health goals, this is a significant barrier. From lack of understanding of their roles to lack of competent mentors and miscommunication with skilled facility leaders, medical directors face many challenges to fully engage in high quality care provision in their facilities.
Dr. Nazir shared his frustrations with Bharath Bynagari, a successful healthcare innovator and the CEO of MavenSphere—a company that has solved many other healthcare challenges. Together they embarked on this mission to solve the teamwork challenge faced by healthcare teams and the result is an innovative program CAREASCEND—a digital nudge platform to enhance teamness among physicians, practitioners and healthcare administrators.

Dr. Arif Nazir is the Chief Medical Officer for Signature HealthCare and the President for SHC Medical Partners a post-acute practitioner company. Previously, Dr. Nazir was a faculty member at Indiana University and excelled in research, education and leadership in the post-acute and long-term care setting. As a recipient of a Geriatric Academic Career Award through Health Resources and Services Administration and as a John A. Hartford scholar, Dr. Nazir led several research projects to evaluate impact of innovative care delivery models for post-acute patients. Most notably, he provided leadership as the medical director of a unique CMS Innovations project, OPTIMISTIC, that significantly reduced the avoidable hospitalizations of long-stay nursing home patients in 19 Indiana facilities. Dr. Nazir is the President-elect of AMDA, Society of Post-Acute and Long-Term Care Medicine and is also the Chair of the Innovations Platform Advisory Council for the association. Dr. Nazir has received many professional awards, most notably the Louisville Business First’s 20 People to Know: Healthcare award in 2018, and the Indiana Hulman Health Achievement Award for Public Health in 2015. He has published his research in several peer-reviewed journals and frequently presents at regional and national platforms on geriatric care issues.
Bharath B. Reddy Bynagari is a serial entrepreneur in healthcare IT. With a mission of- “Innovative solutions with a human touch”, Bharath was the inventor behind Diagnotes, a single platform for Healthcare Communications. Bharath created and developed the health information technology behind Diagnotes. Bharath is president, CEO and founder of MavenSphere Inc, a consulting services company since 2007.
Bharath received a Master of Science in Computer Science from University of Texas at Tyler. Since then, Bharath has more than 22 years of IT and Healthcare experience with Eli Lilly, Stanley Works and First DataBank.
Prasanth is a seasoned leader experienced in bringing Consumer Electronics (CE) products into the market. He’s a recognized expert in guiding engineering teams to deliver complex CE products into the market. Prasanth also worked with enterprise customers to migrating their applications to public Cloud Platforms.
Prasanth has extensive experience in Consumer Electronics industries including Home Automation, Telecom, Wireless, Video and Data communications. He was involved in development of IoT devices, 2G/3G wireless products, Smart WiFi devices, Set-top Boxes, high speed data modems. Prasanth ran Software Development teams for some of the industry giants like Qualcomm, Panasonic, Siemens and STMicro. He has to his credit a patent awarded for his work in WiFi arena.
Prasanth holds Master’s degree in Computer Science and Engineering from Indian Institute of Technology, Bombay, India.

Doctor Michael Wasserman is a geriatrician who has devoted his career to serving the needs of seniors. Most recently he served as Chief Executive Officer for Rockport Healthcare Services, overseeing the largest nursing home chain in California. He previously was Executive Director, Care Continuum, for Health Services Advisory Group, the QIN-QIO for California. In 2001 he co-founded Senior Care of Colorado, which be-came the largest privately owned primary care geriatric practice in the country, before selling it to IPC in 2010. Springer published his book, “The Business of Geriatrics,” in 2016, which details how to succeed in Geriatrics in today’s healthcare marketplace. He previously was President and Chief Medical Officer for GeriMed of America, a Geriatric Medical Management Company located in Denver, Colorado. While at GeriMed, he helped to develop GeriMed’s Clinical Glidepaths in conjunction with Drs. Flaherty and Morley of St. Louis University’s School of Medicine Geriatric Division.
Dr. Wasserman is a graduate of the University of Texas, Medical Branch. He completed an Internal Medicine residency at Cedars-Sinai Medical Center and a Geriatric Medicine Fellowship at UCLA. He spent five years with Kaiser-Permanente in Southern California where he developed Kaiser's first outpatient Geriatric Consult Clinic. Dr. Wasser-man was a co-founder and owner of Common Sense Medical Management (CSM2), a case management company that helped manage high risk beneficiaries of Cover Colorado. He is past chair of the American eriatrics Society’s (AGS) Managed Care Task Force and has served served on the Public Policy Committee for AGS and AMDA. He was formerly a Public Commissioner for the Continuing Care Accreditation Commission. He has recently served on several CMS Technical Expert Panels related to MACRA and Value Based Purchasing.
Dr. Wasserman was a co-founder of MESA (Medicare Experts and Senior Access) a multiyear grant from the Colorado Health Foundation to train primary care physicians in how to effectively care and bill for Medicare patients. He was the lead delegate from the State of Colorado to the 2005 White House Conference on Aging. He also co-chaired the Colorado Alzheimer’s Coordinating Council. Dr. Wasserman has actively supported the Wish of a Lifetime Foundation since its inception and serves on its Board. He served on the Board of The Denver Hospice for fifteen years. He serves as a board member for the American Geriatrics Society’s Foundation for Health in Aging, and is the President of the California Association for Long Term Care Medicine. He has spoken extensively and been published on a variety of topics involving geriatrics, uality improvement, geriatric workforce issues, healthy aging, Alzheimer’s Disease, the business of health care, practice management and managed care.

Before joining Signature HealthCARE, Doug served as CIO and Vice President of Information Technology at Prestige Healthcare and Golden Living, as well as Senior Vice President / Chief Information Officer for Atria Senior Living. Prior to his time at Atria, Doug was co-founder and owner of INTRAcare, a technology company serving assisted living providers, physician practice management groups, and managed healthcare companies. Doug was also Vice President of Information Systems with Alterra Healthcare (now Brookdale Senior Living) in Milwaukee, Wisconsin.
Doug has a Master’s in Science in Software Engineering from Colorado Technical University and a BS in Business Administration from the University of Phoenix. Doug is a member of the Project Management Institute (PMI) and the Healthcare Information Management Systems Society (HIMSS).
